UT Austin must submit relevant forms to the Texas Higher Education Coordinating Board (THECB) for specific kinds of Curriculum Change Proposals.
The Curriculum Change Proposal Qualtrics forms will prompt proposal submitters to fill out and upload the relevant documentation. You can download and complete the forms in advance or download and complete them during the Qualtrics survey.
Curriculum Change Proposals and their THECB Forms
Add a New Degree Program (link to Curriculum Change Proposal (CCP) Qualtrics form)
- Colleges and Schools must always fill out 2 THECB forms for a proposed new degree:
- THECB New Degree Certification Form (“Short Form”) [download link: Short Form]
- THECB New Degree Full Request Form (“Long Form”) [download link: Long Form]
- 30-day notification period to public institutions within a 50-mile radius is required. The Provost’s Office will manage the notification process.
Change an Existing Degree Program (link to CCP Qualtrics form)
- Only certain changes to a degree program require THECB forms:
- Form to increase or reduce semester credit hours for degree program [download link: Semester Credit Hours Change Form]
- Form to change degree program CIP code [download link: Form to Change Degree Program CIP]
- Form to change the name of a degree program [download link: Form to Change Degree Program Name]
- Form to consolidate degree programs [download link: Form to Consolidate Degree Programs]
Add a Certificate (link to CCP Qualtrics form)
- Adding a certificate ONLY requires a THECB form if the certificate has 21-24 semester credit hours
- Certificate Program Certification Form [link: Certificate Program Certification Form]

Change an Existing Certificate (link to CCP Qualtrics form)
- Changing an existing certificate ONLY requires a THECB form if:
- increasing hours from [18, 19, or 20] to [21, 22, 23, or 24] or
- reducing hours required for an existing certificate currently requiring [21, 22, 23, or 24]
