On the form, you will be asked to submit the following information:
- a short summary – not more than one page – of the petition;
- the facts relevant to the grievance, including the name of the administrator or
- administrators who took the adverse action;
- the right or rights the faculty member claims have been violated;
- the redress the grievance seeks;
- a summary of attempts to informally resolve the issue;
- arguments in support of the faculty member’s claim and redress sought; and
- supporting documentation
