The Texas Higher Education Coordinating Board requires the following forms from the University of Texas at Austin in order to approve the corresponding types of proposals.  Colleges or schools should complete the applicable form and email it to the Provost’s Office (  The Provost will be the signing officer on all THECB forms. 

The THECB forms are required in addition to the proposals to change the Undergraduate Catalog which are submitted to the Office of General Faculty. (

Add a New Degree Program

Add a Certificate (If over 20 hours)

  • Certificate Program Certification Form [link]
  • 30 day notification period to public institutions within a 50-mile radius is required.  The provost’s office will manage the notification process.
  • THECB forms are not required for certificates 20 hours or less.

Increase or Reduce Semester Credit Hours

Change CIP Code

Change the Name of a Degree Program

Change Administrative Unit of a Program

Consolidate Degree Programs
